All members of the Applied CS Groups must register for CS-281:Applied Groups
for either 0 or 1 Credit in each semester that they are working in the group.
You can register for 1 Credit in one semester of each of three years. (A
maximum of 1 Credit per year, 3 Credits total.)

1) Name

2) Email address

3) Campus phone

4) Please rank, in order of your interest, the following groups. Rank the
group you are most interested in "1". Indicate equal interest by giving the
groups the same numerical rank.

  ___ CS Content Administration

  ___ CS Pedagogical Tools

___ CS System and Network Administration

___ DBI Group (Database Interfacing Group---WebDB)

  ___ HIP (hardware interfacing project)

  ___ Green Science

  ___ ECS System Administration

5) Are you committed to working an average of about 10 hours each week during
the semester?

6) Are you committed for about 1.5 years for this position?

7) Are you willing to take the initiative to read documentation, learn command
syntax and solve problems with only slight assistance?

(The following three questions are intended to help us determine what
conflicts there may be for your time. Please answer as clearly and completely
as you can. Conflicts do not rule out participation; we simply need to be
able to predict the level of coverage we will have.)

8a) Do you have work study commitments currently? Do you have a workstudy
award? How many hours per week does it permit you to work? Would you give-up
your other work study position if necessary?

8b) Are you planning on going on an off-campus study program?

8c) What extracurricular activities do you participate in? Are there intervals
during the semester in which these activities may preempt your work with the
group?

9) Are you interested in working on campus on applied CS activities during
this coming or another summer? How about during the December/January or March
breaks? (This is not a requirement, merely a statement of interest).

10) If you haven't declared a major yet what is it likely to be? (These groups
are not restricted to CS majors but, with the exception of Green Science,
preference is given to majors or those who intend to be majors.)

11) Have you read the SAGE Code of Ethics, The Earlham Privacy Policy and the
Earlham Acceptable Use Policy? Are you comfortable living with such
strictures? (Please see the attached pages).

12) Check the appropriate spots below. Note that these aren't requirements,
we're just getting a sense of your experience and your future plans.
